Clearance Information
Clearance Information for the 2026-27 School Year
To be eligible to play a sport, your student needs to receive clearance from the Athletic Department. Please complete the following steps:
STEP 1: Complete Online Registration
STEP 2: Pay an athletic fee
Other Athletic Forms:
- Physical Education Credit Waiver
- OSAA Intent To Transfer Certificate
- OSAA Eligible Student Transfer Certificate
- Physical Examination Form - completed by your doctor - (Physicals are good for 2 years; only 1 year for football)
- ImPACT Concussion Permission Form
- Update Existing Physical
